To the Editor: The Commentary by Mr Moore and colleagues1 makes many important and valuable points about the present system of drug safety. Monitoring drug safety without knowing the extent of problems from medicines is like having an FAA that would not have data on airplane crashes. It has been repeatedly documented that problems from medications are grossly underreported.2
