To the Editor: The Commentary by Dr Starfield1 raises a few questions about the efficiency and
effectiveness of US health care spending. It would be helpful to know how
much of the overall cost is used for direct patient services and how much
is used to cover administrative costs, paperwork, corporate profits, advertising,
liability, and other entities that are not directly related to patient care.
