To the Editor: The Clinical Crossroads article by Dr Daley1 and the accompanying Editorial by Dr Eisenberg2 address the unfortunate state of affairs of the quality of service in health care. The solutions proposed by Daley and Eisenberg include the application of complex systems and adaptive theory, incorporation of patients' explicit preferences, and other aspects of patient-focused care. As important as these are, to an extent they miss the point.
