In Reply: Dr Mayer suggests that our criteria
for coronary heart disease (CHD) are arbitrary. In fact, each variable listed
is an established CHD risk factor. The 10-year risk for heart disease with
diabetes is an equivalent to a prior CHD event; a "CHD risk equivalent" according
to the National Cholesterol Education Program Adult Treatment Panel III.1 Our Figure 2, showing increasing event rates with
increasing scores, supports the appropriateness of the weighting.
