In Reply.— Dr Harkness proposes 4 clinical scenarios and suggests that each requires a different therapeutic approach. We agree, and do not believe that practice guidelines should be the final arbiter of treatment choices. In response to his final question, decongestants have not been shown to be helpful and the role of tympanostomy tubes is uncertain and controversial.1,2
