To the Editor: The authors of a Viewpoint described motives for individual physicians and surgeons to specialize.1 It seems to me that they infer that there are too many specialists and not enough generalists. The proper mix of specialist and generalist has been debated for 50 years or more, with the issue most currently focusing on outcomes and quality. Dr Detsky and colleagues overlooked several important features.
