Techniques in Hand and Upper Extremity Surgery premiered in March 1997. In the inaugural issue the editor noted that, despite the number of existing publications focusing on hand and upper extremity surgery, "there is need for a new forum devoted to presenting upper extremity, hand and wrist surgeons with information dealing with the latest surgical techniques, update on the newest equipment, and progress in therapies for rehabilitation."

The journal has some strong and established competitors, including Hand Clinics (WB Saunders) and Journal of Hand Surgery—American and British Editions (Churchill Livingstone). Scandinavian Journal of Plastic and Reconstructive Surgery and Hand Surgery does not focus on the hand as do these other competing journals.
