In Reply: I agree with Dr Kirsch that the issue of physician supply and demand is far more complex than can be presented in a short Commentary touching on many subjects. I also agree that under virtually all scenarios, the United States will need many more US-trained physicians. However, it is possible that major disruptions can occur that would involve the role of physicians vis-à-vis other health professionals, or the development of new technologies that eliminate whole categories of disease, or that would make patient self management a more viable option.
