Low-dose colchicine should be a first-line treatment for either acute or recurrent pericarditis, report investigators of a double-blind study that randomized 240 patients with recurrent pericarditis to colchicine or placebo at 4 hospitals in northern Italy (Imazio M et al. Lancet. doi:10.1016/S0140-6736(13)62709-9 [published online March 30, 2014]). Colchicine effectively treats acute pericarditis and first recurrences, but multiple recurrences pose a major challenge in the management of pericardial diseases.
