To the Editor Intuitively, nephrectomy should confer some risk to the donor. Our major criticism of the study by Dr Muzaale and colleagues1 is that the analysis underestimated the risk of ESRD in donors.

The calculation of cumulative risk over a patient’s lifetime will differ by sex and race. The average lifetime cumulative risk in the study was misleading because the highest proportion of donors was white women, who have the lowest risk.
