Disease management is the current rage among managed care and health care delivery organizations. These programs seek to improve health care outcomes by encouraging use of proven treatments and by empowering patients to take more control over their chronic illness.

All four books in this disease management series are subtitled "Clinical Pathways, Guidelines, and Patient Education." Each book follows a similar outline. Part 1 has several sections: a brief review of the disease and why disease management can make a difference in that condition; guidelines for managing the disease, including complications and special populations; and clinical pathways summarizing the recommendations, which can be used as flow guides or care planning forms.
