Rounds completed, I hesitated returning to my office. Having cited the latest New England Journal of Medicine article about the human papillomavirus type 16 vaccine, an Annals of Internal Medicine article reporting on the use of leflunomide for active rheumatoid arthritis, and a Journal of the American Medical Association piece on dual antiplatelet therapy after percutaneous coronary intervention, I felt particularly brilliant today. I had especially wowed the students and residents when I noted the latest findings in the Lancet regarding the IBIS-1 study demonstrating that prophylactic tamoxifen reduces breast cancer risk among women with high risk. I had deftly introduced this topic while reviewing the case of a woman who had breast cancer 30 years ago and was just admitted with hyperthyroidism.
