Since an early report1 that presaged a global catastrophe, millions of deaths due to an incurable disease now known as AIDS have been reported.2,3 Even in a country with substantial resources to combat the disease, new cases continue to occur.4 However, awe-inspiring progress is evident in the development of powerful drugs5,6 that prolong life in individuals with human immunodeficiency virus (HIV) infection and in the number of programs in place to prevent additional infections and deaths.2,3,79 Such positive developments are tempered by the reality that huge gaps exist in access to effective therapies.3
