In Reply: In response to Dr Krantz, we have previously reported that individuals with structural heart disease have baseline QTc intervals elevated to 413 milliseconds if they have left ventricular hypertrophy, 423 milliseconds if they have had a recent myocardial infarction, 438 milliseconds if they have dilated cardiomyopathy, and up to 460 milliseconds during an acute myocardial infarction. Thus, we agree with Krantz that these patients would be at enhanced risk of exceeding 500 milliseconds with the degree of QTc interval increase we had shown in our healthy sample.1,2
