To the Editor Drs Jobst and Cascino1 emphasized the low number of referrals for epilepsy surgery despite the good outcomes obtained in 2 high-quality randomized clinical trials (RCTs) that compared surgical treatment with pharmacotherapy in patients with temporal lobe epilepsy. But in one RCT, recruitment of patients who had experienced treatment failure with only 2 antiepileptic drugs was so problematic that the trial was halted.2 Enrollment was complete in the other RCT of patients who had experienced treatment failure with 6 antiepileptic drugs.3
