In Reply There have been many well-described epidemics of HIV among people who inject drugs around the world.1 Typically, in addition to limited access to sterile syringes, education, and treatment, a consistent theme in the setting of an injection drug use–driven outbreak is an environment in which there is stronger law enforcement than the public health approach to addiction.
