To the Editor A systematic review concluded that “antibiotic therapy should be initiated within 4 to 8 hours of hospital arrival for patients with radiographically confirmed pneumonia and moderate to high levels of illness severity at presentation.”1 It is unclear from the included studies that this time to antibiotic therapy initiation is applicable to patients with moderate to severe community-acquired pneumonia.
