In reply.—  Dr Smith's firsthand account of the situation in South Dakota provides good examples of both the psychiatric manpower shortage and the need for primary care education referred to in my editorial. Although there may be a surplus of physicians in some specialties, this is certainly not so in psychiatry—particularly regarding the psychiatric needs of the elderly.
