Information Incorrectly Presented and Incorrect Use of Brackets and Parentheses in Equation.

—Two errors occurred in the article entitled "Chance, Continuity, and Change in Hospital Mortality Rates: Coronary Artery Bypass Graft Patients in California Hospitals, 1983 to 1989," published in the July 21, 1993, issue of The Journal (1993;270:331-337). The information in the last sentence of the abstract was incorrectly presented. The sentence should read as follows: "Even with reporting lags and changes in hospital outcomes over time, a policy of channeling high-risk patients away from high-outlier hospitals and toward low-outlier hospitals could lower their [not the] overall risk-adjusted mortality rate by 45% [not 54%]."

On page 332 in the right-hand column, brackets and parentheses were incorrectly used in the equation. The equation should have appeared as follows:
