A 74-year-old woman presented with a 2-year history of painful papules on her upper back and the recent development of similar lesions on her upper arms. Her medical history was significant for a hysterectomy 28 year previously and multiple uterine fibroids.

A family history revealed that the patient's daughter complained of similar painful papules on her face and had also undergone a hysterectomy for multiple uterine fibroids at a young age. One grandson had similar skin lesions.
