My coauthors and I appreciate the positive response to our study on intralesional lipolysis, and we share the opinion of Dr Bechara and colleagues that monotherapy with injections of PTC is unable to completely erase lipomas.1 In some patients, size reduction of grossly 50% of the pretreatment volume offers sufficient relief from the disturbance. The fact that it may be easier for surgeons to remove lipomas after 3 or more intralesional injections of PTC—perhaps by means of the “squeeze delivery” technique—supports the feasibility of this treatment option.2
