In the spring of 1970, nearly 40 years ago, the American Academy of Facial Plastic and Reconstructive Surgery (AAFPRS) held its first international symposium. Of the more than 350 presentations, arguably the most historic one was provided by a lesser known surgeon from Zagreb, Yugoslavia, Ivo F. Padovan, who offered a 10-minute presentation on the “External Approach in Rhinoplasty (Decortication).”1(pp143-146) A revolutionary shot in the rhinoplasty world had been fired, but it was neither heard nor appreciated immediately.
