Within the last 25 years, there have been striking advances in our knowledge regarding those hormonal factors that modulate the longitudinal growth, modeling, and remodeling of bone, as well as the homeostatic control of circulating calcium and inorganic phosphate. We have witnessed the discovery, structural identification, and synthesis of calcitonin, a hormone derived from parafollicular thyroid cells, and the effective clinical application of this agent in Paget's disease of bone, osteogenesis imperfecta, and hypercalcemias of varied causes. Moreover, a sensitive and reproducible assay for circulating calcitonin has been established that is now essential to the early diagnosis of medullary carcinoma in both symptomatic patients and their family members long before the disease becomes clinically manifested. More specific radioimmunoassay systems have been devised for measurements of circulating parathyroid hormone that are currently routinely applicable for the diagnosis of primary and secondary forms of hyperparathyroidism and certain "idiopathic hypercalciuric" disorders characterized by
