Dr Feingold raises an important issue that has, until recently, received relatively little attention. Elderly patients are especially vulnerable to preventable injuries, such as falls,1 during the immediate postdischarge period. In addition, during the transition from the hospital to a nursing facility or home, transfer of information ("handoff") is often faulty and can result in adverse events.
