Dr Walsh raises several issues about "real-world" success rates; readers are referred to a recent, evidence-based reply to Walsh and Penman's prior statement of concern.1 Dr Walsh notes that clinical trial conditions do not perfectly mirror the real world of unaided quitting, and we agree that this is inevitably true. However, Dr Walsh's concerns and underlying conjectures are simply not supported by empirical data.
