We thank Dr Jellinger for calling to our attention his vast experience obtained in his consecutive autopsy series from Vienna. Dr Jellinger divided his subjects into 4 groups: pure AD, mixed dementia (AD and vascular), AD with multiple basal ganglia lacunae, and AD with small infarcts. Although it was not defined in precisely the same way that we defined AD patients with incidental infarcts, I suspect that his cohort of AD subjects with small infarcts most closely mirrors this group of subjects. We also excluded individuals with infarcts in critical locations.
