I am intrigued by your new journal section, "Controversies," and welcome the new dialogue. You selected a difficult initial subject: medical ethics as applied to capitation. Although both discussants seem well qualified to address the logistics of capitated care, the opinion of a true medical ethicist would have lent informed perspective for your readers. Soliciting the opinion of the senior vice-president of a capitation company about the ethics of capitation is a bit like asking a political lobbyist about the ethics of peddling political influence.
