We read with interest the article by Kushner1 on insertion slanting strabismus surgical procedures. In that article, Dr Kushner reviewed the extraocular muscle mechanics and proposed mechanisms of how slanting surgical techniques work.

Dr Kushner mentioned that 2 opposite approaches exist in slanting strabismus techniques: the Bietti method and the Simonsz/von Graefe method. We observed some controversial points that deserve discussion by the author.
