Retinal Tear Related to Air Bag Deployment

A young woman had a facial injury due to air bag deployment during a motor vehicle crash 24 hours earlier. On craniofacial computed tomography, only fractures of the nasal bones were detectable. She presented to the ophthalmic accident and emergency department with a severe headache and visual acuity reduction in the left eye after the crash. Visual acuity was 6/9 (Snellen equivalent 20/32) OD and 6/36 (Snellen equivalent 20/125) OS. Ultra–wide-field retinal imaging showed commotio retinae and deep subretinal peripheral hemorrhage in the right eye and vitreitis and a retinal tear very close to the posterior pole with the retinal edge rolled up in the left eye (Figure). Intraocular pressure measured with Goldmann applanation tonometry was 9 mm Hg OU. B-scan ultrasonography showed that the globe was not damaged. This case shows the potential usefulness of ultra–wide-field fundus images to evaluate the peripheral retina following trauma when indirect ophthalmoscopy might otherwise be very painful and difficult to perform.13
