In Reply Jackson and colleagues note that validating electronic medical records against the patient’s medical file is not commonly done. If progress is to be made, old methods might need to be revised. There is no reason why validation of outcomes cannot be performed by checking the electronic medical records. In addition, the extraction queries can be improved over time. That might mean revising the operative note template to specify not only whether delamination was performed but for what reason.
