—The Cryotherapy for Retinopathy of Prematurity (CRYO-ROP) Cooperative Group, chaired by Earl Palmer, MD, is once again to be congratulated for its continuing contribution of significant information on the natural and treated outcomes of ROP. The Archives publication of the correlation of visual function and retinal structure by Gilbert et al1 confirms that a favorable anatomic outcome as defined in the CRYO-ROP study can have an unfavorable visual outcome, with macular ectopia, and/or macular pigment alteration, and/or optic atrophy as the anatomically identifiable causes.The authors have used the term macular retinal pigment epithelial scarring and have speculated that prior retinal detachment may be a cause of the condition. In 1982, the pigment alterations were reported and called macular pigment epitheliopathy (MPE) with the same possible etiology being suggested.2 In 1986, more information on MPE was published, including the effect on visual function.3 Dr
