Diagnosis: Malleus fixation
The CT scan showed that the head of the left malleus was in contact with the roof of the epitympanum (Figures 1 and 2) and that the anterior process of the malleus was continuous with the scutum (Figure 3). (Figure 4 shows an axial image of the right side for comparison.) The patient subsequently underwent an exploratory tympanotomy, the findings of which confirmed fixation of the malleus. Ossiculoplasty was performed with an incus interposition graft after transection of the malleus head.
