deJong has presented an accurate, unbiased review of the controversy currently surrounding the treatment of spontaneous PLF. The last 8 years have seen more than 80 articles published on PLF in the English-language literature, and we seem no closer now than in the past to agreement on such basic questions as (1) What are the usual symptoms of patients with PLF? (2) What are the usual audiologic findings in patients with PLF? (3) What tests can be done to diagnose a patient before surgery? (4) What are the findings at surgery that confirm the presence of a PLF? (5) What is the natural history of a spontaneous PLF? (6) What are the actual benefits of repairing a suspected PLF?
