Author Missing From Byline and Author Contributions and Affiliations Paragraphs

In the article titled “Fluorescence Identification of Head and Neck Squamous Cell Carcinoma and High-Risk Oral Dysplasia With BLZ-100, a Chlorotoxin-Indocyanine Green Conjugate,”1 published online February 18, 2016, an author, James M. Olson, MD, PhD, was inadvertently omitted from the byline, as well as the author affiliations and contributions.2 The byline, affiliations, and contributions, which should have appeared as follows, have been corrected online:
