A noncommunicating biloculated arachnoid cyst in a neonate was apparently present from birth. This unusual example of intracranial arachnoid cyst was correctly identified by complete neuroradiological study. An important contribution to the diagnosis was made by selective vertebral and carotid angiography via a transfemoral approach. Craniotomy on the 16th day of life afforded verification and definitive surgical therapy.
