To the Editor.—Your editorial in the Archives (106:133, 1973) entitled "Forgotten Firsts" was read with pleasure. In January you published the article, "Pylorojejunostomy as an Alternative to Pyloroplasty," by David Pecora, MD (106:83, 1973) and it confirmed, in my view, the value of "visiting the library." In April 1964 Northwest Medicine published "Gastroduodenal Jejunostomy With Vagotomy" in which I reported an isolated case of a patient operated on Sept 10, 1963, in a manner similar to the above technique. Subsequently, I read A. J. Lesser's article and report on pylorojejunostomy performed with dogs (Surgery 5:837, 1964).

Even though an article has to be rejected by some surgical journals, I was so grateful for the publication of my brief case report by Northwest Medicine, as it allowed me to participate in a small part of the action of the development of new surgical ideas. For this reason, I hope that
