In Reply: In response to Dr Fessel and colleagues, we have analyzed the source of medical care for the 610 HIV-positive men who were seen between October 1, 1997, and March 31, 1998, at visit 28 of the Multicenter AIDS Cohort Study (our article covered 1984 through July 1997). We defined indications for therapy as having a CD4+ cell level of 0.50 × 109/L or less, an HIV RNA count of 10,000 copies/mL or more, or both.1
