To the Editor: Dr Fost1 notes an "emerging consensus" that physicians are at times too aggressive in the treatment of some extremely low-birth-weight (ELBW) newborns. Severely disabled children who are "graduates" of the newborn intensive care unit (ICU) surely provide at least part of the explanation for the increasing skepticism about aggressive treatment of such infants, but there may be another component as well.
