In Reply: There is no question that much uncertainty exists around the role of PSA-based early detection of prostate cancer. However, our position that PSA should not be eliminated as a screening tool was based on several facts. First, population-based modeling data suggest that widespread implementation of PSA screening explains a substantial fraction of recent declines in prostate cancer mortality in the United States.1
