In Reply.—Dr Volpintesta's letter further clarifies why universal health insurance should be placed on the national health agenda and early action taken to implement it. Despite the unexpected action of Congress to provide additional dollars to states to expand health insurance coverage for children, it is likely that the total number of uninsured and underinsured persons will increase, taking into consideration the proximate removal of many from the welfare rolls that currently provide their Medicaid coverage; the increasing costs of private insurance coverage; the continuing disinclination of small employers to provide health care coverage; and the increasing number of individuals who are employed as casual workers.1
