In Reply.—Although Dr Sonis and colleagues suggest that we did not deal with setting as an issue in applicability, their letter clearly demonstrates effective application of our guides to issues in primary care. In fact, we are delighted with the response from Sonis and colleagues. These primary care physicians are doing exactly what we hope other physicians will do: use the concepts of our guide to perspicaciously and critically evaluate issues in using the literature in their own health care milieu. We congratulate them on their astute application of the principles of applicability and recommend their thoughtful comments as an example of how other clinicians can apply our guides to their work.
