In Reply: While Dr Fuller, Dr Vicini
and colleagues, Drs Potters and Freeman, and Dr Sharkey present
thoughtful comments, a few points of clarification are needed. First,
in the "Comment" section of our article, we clearly state that PSA
outcome after the current "standard of practice" of external beam
radiation plus implantation with or without neoadjuvant androgen
deprivation therapy should be prospectively compared with radical
prostatectomy or external beam radiation therapy controlling for the
PSA, biopsy Gleason score, and clinical T-stage. Only with such
comparisons can the "standard of practice" become clinically valid.
