To the Editor Dr Suri and colleagues1 reported data on 1021 patients with flail mitral valve regurgitation and no or mild symptoms and compared outcomes of surgery within 3 months vs a watchful waiting strategy. They concluded that early surgery was associated with greater long-term survival and a lower risk of heart failure than initial medical management. We wish to point out several limitations to the study.
