To the Editor: Dr Lacey and colleagues1 found a significantly increased risk of ovarian cancer in women who used unopposed ERT, but not ERT with progestins. We are concerned that the sample was atypical in that most had a history of suspicious breast lumps. Eighty-four percent of the person-years on which the relative risk estimates were calculated were derived from women who were recommended to have surgery or had surgery for breast lumps.
