Primary Care and Health System Reform

To the Editor: In his Commentary, Dr Kirch1 urged that the Centers for Medicare & Medicaid Services become an agent for health system innovation and reform. I agree with this, as well as his view that academic medical centers (AMCs) can play a central role. However, AMCs first need to become part of the solution rather than part of the problem with respect to primary care, which is the foundation of all health care systems.
