To the Editor: In their randomized controlled trial, Dr House and colleagues1 investigated whether B-vitamin therapy can slow progression of diabetic nephropathy and prevent vascular outcomes. The authors concluded that high doses of B vitamins compared with placebo resulted in a greater decrease in glomerular filtration rate (GFR) and an increase in vascular events. However there are several issues that cause concern in this study.
