A new program to avoid medical errors at all costs—or at least at an initial cost of $7 million in funding by the Agency for Healthcare Research and Quality—is being launched by the agency and the Department of Veterans Affairs.

The Agency for Healthcare Research and Quality and Veterans Affairs' National Center for Patient Safety are collaborating on the program, the Patient Safety Improvement Corps. The goal is to train state health care clinicians during three 1-week sessions on how to prevent medical errors and close calls.
