To the Editor: The Editorial by Dr Fauci1 highlighted the importance of the control of infectious disease and
advocated public health measures coordinated with biomedical research. While
this recommendation is much welcomed, it does not address the mounting evidence
that the effective technologies generated through such studies are not reaching
many of those who need them most.2 A substantial proportion of
this failure can be attributed to health delivery system constraints, a problem
that has undergone fewer systematic assessments than have biomedical interventions.
