To the Editor The DIABOLO trial examined the effect of acetazolamide vs placebo in patients with COPD.1 We would like the authors to respond to the following points.

First, the literature supporting the role of acetazolamide as a respiratory stimulant is scant and hence acetazolamide is not recommended in most guidelines.2
