A Woman With Fever and Lymphadenopathy

A 38-year-old Chinese woman with no medical history presented with a 10-day history of unremitting fever and lower limb and periorbital swelling. She had no cough, dyspnea, or bowel or urinary symptoms. She had no recent travel and no ill contacts. Sexual and family histories were unremarkable. Her temperature was 40°C; heart rate, 100/min; and blood pressure, 130/80 mm Hg. There was bilateral nontender cervical lymphadenopathy and 1+ pitting pedal edema. The remainder of the examination was unremarkable.
